This Satisfaction Survey is designed for Clients/Client Representatives to complete individually or by interview with a staff member – either in person or on the phone.
All Clients are encouraged to provide feedback and are reassures of their privacy when we address their concerns
We encourage all Clients to provide comments in relation to the reason for their satisfaction/dissatisfaction.

If you have an advocate/ person who assists you to carry out your wishes – does our organisation include this person in decision that affect you?

Do you ever feel that staff discriminate against you or treat you badly because you have a disability?

Have you ever had any staff member “abuse” your rights either physically, financially, emotionally or sexually?
